Width of cutout for chassis should be 0.25" wider than chassis for a snug fit, or +0.3125" for some breathing room.

Just because an outside power line runs close does not suggest noise or poor AC from utility. A scope would be userful for examining the AC.

Sounds correct. Typically, the bulb will get bright right at power up, as the reservoir filter fills, then it should fade.

I'm reusing the DV Mark chassis and iron, along with the Marshall PCB from Andrea to build out a "plexi".
Space constraints mean I have to lay the preamp tubes down, and fab a riser for the power tubes.

Welcome to TAG. You didn't mention whether you tested (or replaced) the 1K/5W screen resistors and 5K5 grid resistors, which are normally socket-mounted.

When I have tried stapling piping, my stapler wants to push the piping out of place. In other words, I can't shoot staples close enough to the piping to avoid pushing it out of place as I staple. Advice?

Anyway, yeah, building a one-off amp from a recipe is only the first step! There will always be tweaking after the build is completed, whether to meet the tastes of the builder or the client.
