Previous convention dictated tubes that had beam deflectors internally connected to the cathode were Beam Tetrodes, not pentodes, as they can't be separated. Some designers prefer to have a negative voltage on the suppressor grid, this can only be done with a true pentode. Forming plates are helpful...

Anyone know why the KT tubes require separate cathode resistors? Their tendency to oscillate easily maybe?

the capacitor is styrene I believe.

I've not seen what you describe before. I've seen outputs modulated with line frequency, but not with the input freq. I'm betting on a circuit ground problem. I assume your scope ground is good. Does the percentage of modulation increase with increased gain?

1/2 Amp fuse seems a little small. I'm not surprised it would blow with the turn on surge.

I'm sure space is an issue, but the closer to the associated circuit the better, since your off the card anyway. Pics always critical for best advice!
