Try reverse audio taper. Or wire your audio taper so cut is CCW.

Shepdog is correct. There should not be a connection between that part of the bridge and ground. That would present a direct short to that half of the winding.

You could do something like this, since you're using a dual opamp. The switch will let you flip the phase as desired.

For Eminence, I've used Bernie at BLTSound in the past. He provided good service and his prices were better. I've noticed however, that his prices these days are about the same as everyone else's. Free shipping though, so that helps.

Fixed bias. I guess they kind of split the difference between 6V6 and 6L6. Good solid low end, present clear highs. I'm using the JJ's as well.

Well, the first caveat for anyone just starting out in building tube equipment is that there are potentially lethal voltages inside, so safety first, last, and always. There's a guy who plays at our church on occasion who uses a PDI-03 with his Splawn head and it sounds great. Not sure what's going on inside there, but it's the real deal.

10k will work, but you'll have most of the usable adjustment toward one end of the pot. You could solder a 10k resistor in parallel across the outer legs of the pot to bring the total resistance down to 5k, but that will mess with the taper. Try this. I marked up your drawing showing recommended changes. If it still doesn't get you what you want, you could add an anode follower to one channel to swap the phase and then take each channel to a separate PI input.

No, you'll have to lose the cap across the mix resistor to prevent the high-end rolloff. The mix resistors do load each channel so you won't get as strong a signal hitting the PI. Putting a bypass cap across your mix resistor will look like a low-pass filter to your other channel. Go back to your two 220k mix resistors. You can build your own ground loop breaker, which will still maintain a safety ground, using the attached diagram. If you don't want to build it yourself, you can buy the Ebtech HumX ground loop breaker.
