Need help with grounding connections

I need help understanding exactly how the ground wires should be connected.

I currently have (Using Funk's filter pcb board):

1. pt center tap to chassis
2. solid buss wire running along back of pots, grounded on one end to same chassis point as above
3. B+1 gnd going to 6l6 cathodes
4. B+2 going to chassis gnd (not sure about where this one goes)
5. B+ 345 connected to buss wire behind pots near where preamp cathode caps are grounded.

Input jack is not grounded to chassis nor is spkr output. Tried touching these to ground but didn't reduce hum.

Do I need to connect all of the pcb grounds together and run a wire to chassis?
