There is a full Gerber file you can download. Upload it at jlcpcb.com, and for like $20 you can get 5 PCB's made. Buy the parts to populate the board, and for good Karma, maybe offer the remaining boards to other members here for the cost of postage.

I've used this circuit, and it's very simple and easy to use.

Re: voltage (From Lauri): Maximum recommended supply voltage for 40175 is 15V. V+ can be anything from 5V to 15V

From my project notes:
  • GND - All grounds connect to the same trace
  • V+ - All V+ connections are the same trace
  • CTRL 1-4 - Connects to Relay coil anode (-)
  • V+ and GND nearest the IC are the main connections to power and ground
  • D1-4 connect to one side of each momentary switch
  • V+ - near each corner, connect to the other side of each momentary switch
  • D6-9 - connect to each channel’s LED indicator

BOM
  • R1 - 10k
  • R2 - 10k
  • R3 - 10k
  • R4 - 10k
  • R5 - 10k
  • R6 - *
  • R7 - *
  • R8 - *
  • R9 - *
  • C1 - 10n
  • C2 - 100n
  • D1 - 1N4148
  • D2 - 1N4148
  • D3 - 1N4148
  • D4 - 1N4148
  • Q1 - BC547 (or 2N7000, but reverse the orientation. Pinout is opposite)
  • Q2 - BC547
  • Q3 - BC547
  • Q4 - BC547
  • U1 - 40175

LED Resistor Values

  • Red - 150 Ω
  • Green - 145 Ω
  • Blue - 70 Ω
  • Yellow - 145 Ω
  • Orange - 140 Ω
  • White - 70 Ω
  • UV - 165 Ω
12V (for use with 12V relays)
  • Red - 500 Ω
  • Green - 495 Ω
  • Blue - 420 Ω
  • Yellow - 495 Ω
  • Orange - 490 Ω
  • White - 420 Ω
  • UV - 515 Ω
