If you are running 12V relay supply, that relay should work.
For the FET input jack, Switchcraft type 13E .
2N4416A is an equivalent JFET.

Angelus; Nice pics of your progess. One detail thing: I don't know what you used to draw the wire layout on the signal board, make sure you wipe it out. Some materials(like pencils) have conductive characteristics.
