I think you have pin 1 of V1 connected to the wrong end of the 220k plate resistor. That explains the too high voltage on pins 1 and 3 of that tube (and the 'almost' no sound).

The circuit looks different enough to consider it not an AC30 Top Boost (or Rocket) type circuit. AFAIK, the AC30 Top Boost and Rocket have 2 gain stages then a cathode follower driven tone stack and the tone stack is different (1M pots and a 50pF treble cap among other differences). The MM iron is very good in my experience and I have purchased enough that the quantity discount makes them competitive. I just checked my three 12AX7s on my VTV tester. One is higher gain than the other two but the triodes are not as well DC balanced, so that goes in V1. Just my opinion but compared to the cost of the entire amp, the extra cost for MM trannies isn't that huge. And if you're trying to build the absolute best amp you can, why not get the best? Is the 100k in series with the trimmer? And is that a 1M trimmer?
Now that I think about it, I don't think I've ever had my 500k-A HRM bass control as high as 5. Next time I'm in there I might try a 470k-1M in parallel with it.

Shouldn't be getting any noise whatsoever when on Standby. Should be dead silent. Are you sure the Standby switch and power supply are wired correctly? Measure voltages while in Standby, you should get heater voltage and DC voltage up to the Standby switch but no voltage anywhere else. OT center tap...
